Overview of the Surgery

We are a 3 partner training practice, working from purpose-built premises in the pleasant mid Norfolk market town of East Dereham. Just twenty minutes drive from Norwich, we are also within easy reach of Cambridge, Bury St. Edmunds and Ipswich. The practice is currently part of South Norfolk CCG.

Our surgery team comprises a full complement of ancillary staff, together with attached Community Nurses, Midwives and Health Visitor. We also have an Emergency Care Practitioner working with us for four days each week offering in-house appointments and home visits. Mid Norfolk Locality employs a team of three Nurse Practitioners to focus on the clinical needs of the elderly in local care homes and those in their own homes. We have one of the team, a Prescribing Nurse Practitioner - based at Theatre Royal Surgery and she undertakes home visits as well as providing regular support to the three care homes that our practice looks after.

We are fully computerised, using TPP SystmOne network with integrated Appointment System together with an extensive suite of ancillary Microsoft (and other) programmes. Our Financial Team uses Sage Accounts and Iris GP Payroll programmes. The practice operates registration, item of service and pathology service links and is comprehensively furnished with up to date office and clinical equipment.

Our Practice currently uses IC24 for out of hours services between the hours of 6.30 p.m. and 8.00 a.m. weekdays and all weekend.

Our patient list of approximately 9,000, falls within a fairly compact area - extending to an approximate 4.5 mile radius of the town centre. The practice is open every weekday between 8.00 a.m. and 6.30 p.m. Throughout the day there is always a nurse on duty and they are able to assess any emergencies which may arrive when there is no doctor on the premises. The daily duty doctor can be contacted by mobile telephone throughout the day. Our practice list is currently closed to new registrations.

We are a training practice and we regularly have medical students in the practice as a result of the collaborative work between ourselves and the UEAMedicalSchool. From time to time we also have trainee practice nurses or administrative staff working on the premises. From spring 2017 we will have two of the new cohort of Physician Associates based in our practice as part of the UEA's new training programme. In addition we also train FY2 doctors and currently have a doctor on placement with us until February when his replacement will start work with us.

All staff and partners are encouraged to attend external courses and we hold our own in-house sessions on a regular basis for all staff. In past years we have held five residential training weekend attended by all members of the practice team.

Since April 2004 we have offered a range of enhanced and private services from our premises and we opened a dispensary in May 2004 that enabled us to dispense to approximately 1000 of our patients (depending upon where they live in the practice area). The dispensary was superceded by a Pharmacy in 2011 and this is operated by an external company and open for one hundred hours per week.

We had our first CQC inspection in March 2014 and were randomly selected from other South Norfolk CCG practices to have a second, new-style CCG inspection in October 2014. Both inspections were passed with flying colours and were a reflection on the excellent staffing, facilities and systems that are in place at Theatre Royal Surgery.

We work closely with our colleagues in the Mid Norfolk Locality of South Norfolk CCG and are currently developing an MCP model of care based on the successful ICO pilot that we designed a few years ago.

We feel that we are a happy, efficient and caring practice where all members of the team are encouraged to use their initiative and particular skills to ensure that we offer the highest possible standard of care to our patients.

Minimum of 5 years post registration experience with at least 2 years in primary care/A&E/OOH setting.

Job Description - Nurse Practitioner / Minor Illness Nurse

Summary

We are seeking an experienced nurse, who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within our practice. The successful applicant will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required.

In order to work at this level, NMC requirements for advanced practice must be met in that the post holder will work to their personal scope of practice and competencies, seeking advice where and when appropriate with clinical colleagues, GP's or other professional agencies.

Scope and Purpose of the Role

To deliver a high standard of patient care as Nurse Practitioner (NP) in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

To manage a clinical caseload, dealing with the presenting patient’s needs in a primary care setting, including triage with the duty GP.

To support the Lead Nurse and the nursing team and ensure the highest standards of care are provided for the patients.

To support the development of nursing services offered by the practice

To provide clinical mentorship to the nursing team within the practice.

To undertake clinical visits to patients in their own homes or our practice patients residing in care homes.

To work collaboratively with clinical staff within the locality to deliver integrated care as directed by the GP partners.

To assist in the development of referral pathways to secondary care, AQP and diagnostic and treatment centres (where relevant).

Primary Duties and Areas of Responsibility

Clinical

The post-holder will:

Provide the choice of direct access to a NP, both in the Practice and over the telephone, for the patients of Theatre Royal Surgery.

Make professionally autonomous decisions for which he/she is accountable.

Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan or further referral to alternative practitioner.

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports

Where the post holder is an independent prescriber, to prescribe safe, effective and appropriate medication as defined by current legislative framework (Surgery/National policies)

Provide safe, evidence-based, cost-effective, individualised patient care.

Offer an holistic service to patients and their families, developing (where appropriate) an on-going plan of care and support, with an emphasis on prevention and self-care.

Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.

Process correspondence and patient related data received into/within the practice, whether by post or electronically in line with practice protocols.

Provision of enhanced services and minor operation procedures as required.

Refer patients directly to other services/agencies as appropriate in line with Practice policy.

Support the practice team in initiating/signposting and assisting in the development and delivery of specialist primary care services, such as long term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease, hypertension and CKD).

Work with nursing, clinical and health care assistant colleagues to ensure that National Service Frameworks (e.g. Coronary Heart Disease/Older People/ Diabetes /Mental Health) are being delivered

Work with mutli-disciplinary team within the Practice, and across the wider Mid Norfolk Locality and Clinical Commissioning Group (CCG), to promote integrated and seamless pathways of care.

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.

Participate in identification of community health needs and develop patient/family-centred strategies to address them.

Help develop and set up new patient services and participate in initiatives to improve existing patient services.

Be aware of and contribute to planning and delivery of CCG patient pathways and, if required, be pro-active in the development and provision of new clinical services within the practice and the locality.

Teaching and Mentoring

The post-holder will:

Promote a learning environment for patients, nurses and other health professionals.

Where appropriate, contribute to the planning and implementation of the teaching for existing staff, such as medical students, NP students, GP registrars, Health Care Assistants, within the Practice.

Professional Role

The post-holder will:

Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.

Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review.

Maintain their professional registration.

Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.

Work towards nurse revalidation requirements. within the latest NMC Code of Professional Conduct.

Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards.

Keep up to date with pertinent health-related policy and work with the practice team and, where appropriate, with the wider locality team, to consider the impact and strategies for implementation.

Work collaboratively with colleagues within and external to the practice.

Demonstrate appropriate leadership.

Pro-actively promote the role of the NP within the Practice and externally to key stakeholders and agencies.

Administrative Role

The post-holder will:

Encourage and develop teamwork within the practice.

Participate in multi-disciplinary protocol and Patient Group Directions development.

Liaise with the Lead Nurse to identify appropriate opportunities to delegate both clinical and administrative tasks to more appropriate staff.

Help the practice operate in a cost-effective manner.

Work with the Lead Nurse to identify and manage nursing care risks on a continuing basis.

Participate in practice meetings as required.

Participate in audits and inspections as appropriate.

Work closely with the practice team in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets.

Health And Safety, Risk Management, Infection Control and Other Policies

The post-holder must comply at all times with the Practice’s Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.

The post-holder will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990).

Post-holder will have indemnity cover either by self or via the working organisation in accordance with working level.

The post-holder will comply at all times with the Practice's Infection Control, Equality & Diversity and Confidentiality policies and any other policies that are in place at the Practice for the protection of clinicians, staff, patients and the general public.

Communication and Working Relationships

The post-holder will be expected to communicate with staff at all levels throughout the organisation as well as patients and carers.

Of particular importance is the development and maintenance of effective communication and working relationships with, and between, external agencies and secondary care clinicians.
