1)National Governors Association Request for Applications-Rural Health Learning Collaborative

2)Coordinating Efforts to Enhance Hospitals’ Role in Population Health- Proposal Deadline Extended

The Robert Wood Johnson Foundation is seeking proposals from organizations who can serve in the role of a coordinating office to enhance hospitals’ role in population health. Rural Health Valuerecently released new resources focused on helping rural healthcare organizations move toward value-based care:

Demonstrating Critical Access Hospital Value: A Guide to Potential Partnerships

Using a market based approach, this guide assists CAH leadership in identifying ways to demonstrate the value they bring to potential partners including networks, affiliations, payers, community-based organizations, or accountable care organizations.

UDS Crosswalk to Quality Reporting Programs

Intended to help health centers identify which UDS measures are being utilized by other programs, such as CMS Accountable Care Organizations (ACOs) and the new CMS Quality Payment Program (QPP). Although FQHCs and RHCs are exempt from QPP, they are encouraged to voluntarily submit quality data for the program. 4)RTT Collaborative Annual Meeting- April 19-21

Registration is now open for the RTT Collaborative Annual Meeting in Anderson, SC, April 19-21, 2017. Educators and trainees in both undergraduate and graduate rural education and training programs of all types are encouraged to attend. A limited number of $1,000 travel and meeting stipends are available on a first come/first register basis.Click herefor more information.

5)Hillman Innovations in Care Program

The Hillman Innovations in Care (HIC) Program is a multi-year initiative that supports the advancement of leading-edge, nursing-driven models of care that improve the health and health care of vulnerable populations that encounter barriers to accessing health care services. 6)National Association of Community Health Centers’ Annual Community Health Institute

If you are conducting research with health centers and/or have innovations that could be replicated in health centers, you are encouraged to submit to the National Association of Community Health Centers’ Annual Community Health Institute (CHI).The CHI brings together health center clinicians, executive directors, State and Regional Primary Care partners, board members, and advocates.This conference provides opportunities to share research and innovations, find other health centers with similar needs and interests, and facilitate partnerships.The conference will take place August 27-29 at the Manchester Grand Hyatt in San Diego, CA.
