2016 Scholarship Application
The Tiger Fan Club is pleased to announce that it is offering two $1,000.00 scholarships and two $500.00 scholarships to senior students from Farmington High School. These scholarships are designed to celebrate the participation of students in extracurricular sports or activities while simultaneously excelling in the class room and community. The Tiger Fan Club Scholarships reflect an appreciation for those students who have demonstrated how preparation and commitment lead to success.
(1) $1,000.00 scholarship and (1) $500.00 scholarship will be awarded to a male FHS student.
(1) $1,000.00 scholarship and (1) $500.00 scholarship will be awarded to a female FHS student.
Criteria for Tiger Fan Club Scholarships
ü Current full-time (12th Grade) students at Farmington High School.
ü Participated in and in good standing with FHS sponsored athletic sports or activities that are member groups of the 2016 Tiger Fan Club, during grades 9 – 12 (participation does NOT have to be as a letter winner).

o Football
o Marching Band
o Basketball, Girls
o Hockey, Boys
o Hockey, Girls
o Cross Country, Boys
o Cross Country, Girls
o Lacrosse, Boys
o Lacrosse, Girls
o Swimming & Diving, Boys
o Swimming & Diving, Girls
o Volleyball, Girls
o Track & Field, Boys
o Track & Field, Girls
ü Acceptance into any post-secondary school as a full time student, i.e., four year college, community college, trade school, technical college
ü No violations of Minnesota State High School League’s (MSHSL) Student Code of Conduct on record.
ü Complete the attached information packet, including:
o List the FHS sport(s)/activities that you were/are a registered participant
o One written reference from a teacher (form attached)
o One written reference from a coach or activity director (form attached)
o Write and attach a School and Community Related Activities Resume (a listing of involvement in activities in school and your community during grades 9 – 12. This is your chance to shine! Feel free to brag about all you’ve done and honors you’ve received during your high school career.)
o Write and attach a 100-500 word essay entitled “What Athletics / Activities Has Done to Prepare Me for My Future.” Share with us some of the reasons you have participated in athletics or activities, and how your experiences have helped shape you. Let us know what you learned or have taken away from your participation, and how the experience may help you in the future.

PLEASE NOTE:
· Scholarship funds are allocated from TFC fundraising associated with FHS sports and activities.
· All applications must be returned to the FHS Guidance Office no later than 12:00 noon on April 29, 2016.
· Athletics / Activities participation and violation-free verification will occur after that time.
· The Scholarship Selection Committee will not consist of any Tiger Fan Club Board members.
· The Scholarship Selection Committee will consist of an FHS Administrator, FHS Teacher, FHS Counselor, a District 196 School Board Member and a Farmington Business Person.
· Scholarship applicant names will be redacted and replace by an applicant number after verification of eligibility by FHS.
· The selection committee will be basing their decision on the merits of the application without knowledge of who the applicant is.
· Scholarship winners will be announced at the FHS Senior Awards Banquet on Wednesday June 1, 2016.
· Scholarship monies are available after the winner presents verification of their first term attendance at the post-secondary institution of their choice to the Tiger Fan Club through the FHS Athletics and Activities Office. Verification might include a copy of grades, letter from registrar, campus housing receipt or similar proof of attendance or school related expense that confirms attendance.
