Departmental Overview: / Health and Aged Services has a workforce of approximately 200 staff involved in service delivery, strategic and service planning, coordination and policy roles. The Health Services team works to promote and maintain high standards of public health and wellbeing, with a focus on disease and illness prevention, with key service responsibilities covering Food Safety, Public Health and Immunisation and an important role in Council’s Emergency Management Plan. Aged and Disability Services undertakes planning, policy, advocacy, partnership and service delivery roles in providing quality home and community support services for older people, people with a disability and their carers and in the development of strategic plans for our older community. Banyule City Council Aged and Disability Services is committed to providing inclusive services. Position Objectives: / To lead the development and operation of high quality and innovative Social Support Programs, and liaise with community social support and older persons’ groups across the local area.

Key Responsibility Areas: /

Strategic Management

  • Contribute to the development and implementation of strategic and business plans to achieve financial, quality management, client service and best practice objectives in aged and disability programs.
  • Provide input into the overall planning, policy development and co-ordination of related social support programs.
  • Develop and analyse appropriate management information and statistical recording systems.
  • Provide advice on current trends in Programs and assist the development of new programs in areas of need.
  • Contribute to the development of plans and policies for the Aged Services Unit.
  • Recommend changes and participate in service reviews to continuously improve service delivery.
  • Incorporate key Health Promotion principles into service planning.

Operational Management

  • Develop and maintain effective communication mechanisms within the program area
  • Manage Social Support resources to achieve organisational goals and performance measures within budgetary guidelines.
  • Design, coordinate & implement appropriate individual service arrangements that are relevant to the assessed needs of the client, in consultation with the client and/or carer.
  • Contribute to the preparation of submissions and tenders designed to enhance services for clients & maximise Government funding for the program areas.
  • Provide advice or information about day to day issues within the Service.
  • Participate in local and regional committees dealing with Services.
  • Develop, maintain and improve quality management systems including food HACCP.
  • Develop and maintain internal and external partnerships to enhance service delivery.
  • Service promotion activities which demonstrate the benefits of the Social Support program to staff,directors, councillors, participants and the general public.
Assessment and Care Planning
  • Arrange home visits with clients to ensure timely and effective assessments and reviews are conducted for Planned Activity Group services.
  • Using the mandated suite of Service Coordination tools, and Council’s adopted assessment tool, undertake service specific assessments, reassessments and reviews to determine eligibility and priority of access to HACC services according to Council guidelines.
  • In consultation with the client and/or carer, develop appropriate care plans outlining care objectives and individual service arrangements relevant to the assessed needs of the client.

Staff Management

  • Demonstrated ability and experience in Staff Support and supervision.
  • Manage contracted staff and labour hire agreements in consultation with the Workforce Development Officer
  • Ensure that all staff have a thorough understanding of their goals and Council’s Working Together Working Better document.
  • Conduct annual staff development reviews for Social Support Program Leaders and Program Workers, Assisted Transport Drivers and Program Volunteers.
  • Support the development of consultation and communication strategies to ensure that staff and volunteers are valued for their contribution to the team and have an opportunity for feedback regarding their role both individually and at team meetings.
  • Implement corporate policies in relation to personnel/employment practices.
  • Co-ordinate and monitor staff wellbeing and ensure OH&S issues are identified and managed in a timely manner.

Location
Due to operational requirements the incumbent may be required to rotate across any of the managed Social Support Program or service centres located within Banyule City Council.
Organisational Relationships: / Position reports to:Coordinator HACC Service Delivery
Supervises:Social Support Program Leaders, Social Support Program Workers, Assisted Transport Drivers and Program Volunteers
Internal Liaisons:Employees at all levels of Council on issues pertaining to the operation of the Service
External Liaisons:Business and community groups, Government departments and agencies, other service providers, staff associations, peak bodies, customers, suppliers and individual members of the community.
Accountability and Extent of Authority: /
  • The Team Leader, Social Support is responsible for the provision of comprehensive, responsive and high quality programs which meet the needs of the residents of the City of Banyule, within defined resources.
  • The Team Leader will participate in the formulation and implementation of policies, procedures and guidelines.
  • The Team Leader has delegated authority to monitor and authorise the Unit’s Budget expenditure within organisational guidelines.
  • The Team Leader will provide direction and support to the staff, participate in staff recruitment, training and development in consultation with the Workforce Development Officer, and will appropriately delegate tasks and functions.

Judgment and Decision Making: / The Team Leader will be required to:-
  • provide authoritative advice and make recommendations to the Co-ordinator
HACC Service Delivery on issues associated with Social Support.
  • provide informed and effective decision-making particularly in situations requiring an urgent and immediate response.
  • ensure the incumbent operates in a broadly regulated environment, receiving limited day-to-day supervision. Supervision is generally in the form of pre-determined objectives and guidelines or in the form of special tasks, projects or assignments.

Specialist Knowledge and Skill /
  • Experience in managing the provision of social support activity programs & knowledge of the Aged & Disability service sector.
  • Experience in Assessment and Care Planning.
  • An understanding of the physical, social & cultural needs of frail elderly people & people with disabilities or dementia.
  • Experience in the use of relevant computer software and ability to use and interpret statistical and financial reports and budgets.
  • Demonstrated interviewing, care planning and assessment skills in Aged Care.

Management Skills /
  • Ability to implement change strategies and to manage in an environment of resource constraints.
  • Ability to manage and prioritise a variety of complex tasks concurrently with minimal supervision.
  • Experience in management of staff and their programs, including identifying performance indicators, developing and implementing systems for ongoing appraisal and support and implementation of relevant staff training and development programs.
  • Commitment to consultation processes and capacity to liaise and negotiate with community groups.

Interpersonal skills: /
  • Excellent written and verbal communication skills.
  • Strong people management, leadership, negotiation & conflict resolution skills.
  • Demonstrated experience in working within quality management systems.
  • Demonstrated ability to complete financial andstatistical returns.

Qualifications and experience: /
  • Tertiary qualification in human services or related field and several years relevant experience.
  • Appropriate qualifications relating to Assessment & Care Planning in Aged Care.
  • Current Victorian Drivers Licence.
  • Current National Police Check and Working with Children Check.
